Job Description:
Position ConceptUnder the direction of the supervisor, the
Foster Care Case Manager II will work with foster children, their
family/guardian, and foster parents to ensure the provision of
comprehensive case management services. Must be able to conduct
intake placements, advocate for clients in court, conduct home
visits, assess needs of children and families, transport clients as
required, and complete required paperwork. Must be able to manage
crisis situations as they arise.Essential Functions* Respond to all
calls requesting placements and screen for appropriateness.*
Complete Intake information packet.* Confirm PERCS foster parent
availability and arrange for pick-up.* Monitor transfer of
child(ren) from natural parent/guardian to Foster Parents.*
Maintain regular contact with the Foster Parents.* Maintain
daily/monthly documentations.* Arrange and monitor discharge of
placements.* Arrange for visitation with children as required
according to licensure standards.* Arrange for consultation with
foster parents as required according to licensure standards.*
Update ITRS.* Maintain and update PCPs.* Maintain contact with
child and family team members.* Attend child and family team
meetings; attend IEP meetings.* Assist with or conduct training
workshops as assigned; assist with child care during training's.*
Be available during the work week for crisis calls with your
consumers and families.* Be available to all Foster Parents while
serving a rotating weekend on call shift.* Attend court, DSS, or
School meetings as necessary.* Keep MIS system updated in a timely
manner.* Complete critical incident reports.* Complete QMHP service
notes.* Prepare notes and needed paperwork for foster parents as
required.* Conduct a weekly consultation with the consumer.*
Conduct a monthly consultation with foster parents.* Monitor
medications and medication administration records.* Attend medical
evaluations as needed.* Staff Responsibilities include; Attend
weekly supervision with Program Supervisor, Attend monthly
individual supervision, Attend two times a month case supervision,
Attend Program Team meetings, Attend community meetings when
needed, Successfully complete all required agency
trainings/in-services, Complete all required paperwork in an
accurate and timely manner, Conduct monthly reviews of files to
ensure completeness.Other Duties Include, But Not Limited To*
Participate in training and development activities as required.*
Report any acts, incidents or conditions that reflect the
possibility of inappropriate youth-to-youth or staff-to-youth
relationships.* Report any use of physical force and all unusual
incidents per Eckerd policy and state guidelines.* Perform other
duties as assigned.Position SpecificationsEducation: BA/BS degree
from an accredited College or University in a Human Services
related field of study.Experience: Two years' experience in a Human
Services/Mental Health field. Must meet requirements to be a
Qualified Mental Health Professional (QMHP) under North Carolina
standards. Skills: Excellent interpersonal, written, and verbal
communication skills. Skilled ability in computer systems including
PC software, spreadsheets, Word, Access, PowerPoint and other
applications. Detail oriented and able to manage multiple
assignments independently. Knowledge of state and federal laws,
regulations and practices related to Family foster Care Program.
Ability to maintain good rapport with community partners and
supports, other employees, and demonstrate constructive
problem-solving skills.Physical Demands: Working conditions are
normal for office environments and meetings in community-based
settings. Sitting, standing, walking and occasional lifting of up
to 20 pounds. Operating a personal computer is a routine part of
the job. Incumbent must be able to travel to outlaying offices and
offsite meetings. The work schedule fluctuates according to the
demands of the position.Other: Must meet state criminal background
check requirements.Travel: Must be able to meet requirements for
Eckerd's Auto Insurance and be able to drive for business purposes.
Incumbent must possess and maintain a valid NC driver's license and
have reliable transportation. Required to travel frequently between
office and locations within the service area, attend meetings and
conferences required to fulfill the obligations and
responsibilities of the position.
