Job Details Level Experienced Job Location Asheville, NC - Asheville, NC Position Type Full Time Education Level 4 Year Degree Salary Range Undisclosed Travel Percentage None Job Shift Day Job Category Health Care Description Role Summary Works with interdisciplinary team to identify and implement best practice patterns across the continum that provide optimal patient care, efficient resource utilization, and top performance for patient satisfaction. Maximizes positive financial outcomes for designated patient populations for intervening at key points in the disease process. Facilitates patient discharges, advocates for patients and families, and helps reduce length of stay. Essential Responsibilities Identifies and implements best practice patterns across the continuum for specific high cost/high volume patient populations that optimize quality care, utilization, reimbursement, and length of stay. Analyzes data related to diagnosis for financial, performance improvement and patient satisfaction indicators to identify opportunities for improvement Requests data related to clinical issues, identifies trends and reviews current literature and provides information related to best practice patterns to the service line and staff. Leads teams in the development, supervision and updating of the discharge plan Identifies and facilitates optimal outcomes through best practice patterns. Supports the implementation of best practice and efficient delivery of care at the bedside. Provides education to the physicians and nursing staff for optimal patient care/outcomes. Develops or makes available educational materials for patients and families to become more knowledgeable about their disease process and are able to participate in their plan of care. Acts as a resource for staff to help patients and families meet desired outcomes. Collaborates and consults with interdisciplinary team members to achieve desired outcomes Attends multidisciplinary rounds. Collaborates with all members of the Health Care Team to achieve optimal outcomes. Provides and documents individualized patient and family education Works closely with Medical records staff involving concurrent coding and LOS Completes other duties to support Asheville Specialty Hospital as assigned or requested Collaborates and consults with interdisciplinary team members to achieve desired outcomes Attends multidisciplinary rounds Collaborates with all members of the Health Care Team to achieve optimal outcomes. Qualifications License: Current RN license with NCBON Must have and maintain current BLS Experience: A minimum of three years recent, relevant clinical hospital experience as a RN Preferred Skills, Knowledge, and Abilities Education: BSN or MSN Certification in related field Experience: At least one-year hospital experience in ICU, or step-down unit. At least one year hospital Case Management experience.
