Currently seeking an experienced Home Health RN Case Manager for
coverage between Worcester and Milford, MA.
Monday-Friday schedule with limited on-call/weekends, competitive
pay, mileage reimbursement, and 13-week contracts.
Manage a caseload of patients from admission to discharge,
supervising LPNs and Home Health Aides.
Require minimum 2 years of recent Home Health RN Case Management
experience with OASIS visits in a Medicare certified agency.
