Job Description:
Trusted Ally Home Care is seeking a dedicated and experienced
Senior Case Manager to join our compassionate team. We specialize
in delivering personalized home healthcare and benefits
coordination services to former nuclear and uranium workers under
federal health programs. In this role, you will oversee case
management processes, ensuring the delivery of excellent patient
care while adhering to federal regulations and company policies.
Coordinate comprehensive care plans for patients to ensure all
health needs are met. Manage a team of case managers and healthcare
professionals, providing training and development to ensure
high-quality service delivery. Liaise with federal agencies,
healthcare providers, and other stakeholders to facilitate
effective communication and program compliance. Monitor and report
on patient progress and program outcomes, adapting strategies as
needed to meet program goals. Ensure strict confidentiality and
privacy of health records in compliance with federal and state
regulations. Assist in the development and implementation of policy
and procedure changes to improve service efficiency and quality.
Strong leadership and team management skills. Excellent
organisational and communication skills. Proven ability to
negotiate and advocate for patient needs. In-depth knowledge of
federal health programs and regulations, particularly EEOICPA and
White Card Program. Critical thinking and problem-solving
abilities. Medical Insurance Dental Insurance Vision Insurance 401k
Certification in Case Management (CCM) highly regarded. Licensed
Registered Nurse (RN) in the state of practice, if applicable.
