The ideal candidate will have a minimum of 5 years of hospital case management experience and proficiency in Cerner/PowerChart and discharge planning platforms. Key Responsibilities /t - Manage a caseload of approximately 25–28 patients. /t - Perform comprehensive patient assessments and develop individualized discharge plans. /t - Coordinate care across interdisciplinary teams to ensure appropriate resource utilization and timely discharges. /t - Facilitate communication between physicians, patients, families, and post-acute care providers. /t - Complete PASRR (ASCEND) entries and ensure compliance with state and federal regulations. /t - Utilize Cerner/PowerChart and WellSky (formerly NaviHealth) for documentation and care coordination. /t - Apply PAE knowledge and ensure accurate documentation. /t - Float within scope of practice as needed to meet facility requirements. Requirements /t - Experience: Minimum 5 years of hospital case management experience (Required) /t - EMR: Cerner/PowerChart experience (Required) /t - Experience with WellSky (formerly NaviHealth) (Required) /t - PASRR entry experience (ASCEND) /t - Knowledge of PAE processes /t - Active RN license (Pending license not accepted) /t - First-time travelers will be reviewed /t - Local candidates considered on a case-by-case basis Schedule & Additional Details /t - No weekend requirement /t - No on-call requirement /t - Block scheduling not available HonorVet Technologies Job ID 31878700.
