This role is pivotal in providing comprehensive nursing care to elderly patients, ensuring they receive coordinated healthcare that aligns with our mission to improve human life. By joining our team, you will be part of a network that focuses on high-quality, patient-centered care across nearly 2,000 sites nationwide, including specialized PACE centers designed to meet the unique needs of older adults. Assess, plan, implement, and evaluate nursing care plans for elderly patients in a PACE setting. Collaborate with a multidisciplinary team to facilitate integrated care tailored to the individual needs of patients. Administer medications, monitor vital signs, and document patient progress. Educate patients and families on health care management and preventive measures. Participate in continuous quality improvement projects and professional development activities. Strong clinical skills with a focus on geriatric care Excellent communication and interpersonal skills Proficiency in electronic health record systems Ability to handle stressful situations calmly and effectively Experience in administering medications and managing multi-dimensional care plans Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active Registered Nurse (RN) license in the state of practice Basic Life Support (BLS) Certification Preferred: Certified Gerontological Nurse (CGN)
