Job Description:
HCA Healthcare is seeking a dedicated Registered Nurse (RN) for
our PACE (Program of All-Inclusive Care for the Elderly) location.
This role focuses on providing comprehensive healthcare services to
elderly patients, managing their long-term care needs and
coordinating between a variety of healthcare professionals to
enhance their quality of life. The RN will play a key role in
assessment, planning, facilitation, and advocacy for options and
services to meet participants’ health needs. Conduct comprehensive
assessments to establish a personalized care plan for each
participant. Facilitate inter-team communication to ensure accuracy
of patient records and effectiveness of treatment plans. Monitor
patient health status and respond to changes in condition. Educate
participants and family members on health care issues. Administer
medication and conduct routine medical procedures within the scope
of practice. Clinical nursing skills Knowledge of chronic disease
management Patient and family education Ability to work effectively
in a team Excellent communication and organizational skills Medical

Valid RN license BLS/ACLS Certification Certification in
Gerontological Nursing preferred
