Job Description:
HCA Healthcare seeks a dedicated Registered Nurse Case Manager
for our Home Health Division, tasked with providing exceptional
patient-centered care across our community-based services. This
role involves managing patient care plans, coordinating with
healthcare professionals, and ensuring that patients receive
comprehensive services tailored to their individual needs at home,
in line with our commitment to improving human life. Your clinical
expertise, compassionate approach, and problem-solving skills will
support our mission and enhance our community services. Assess
patient health needs and develop comprehensive care plans.
Coordinate with physicians, nurses, and other healthcare
professionals to ensure the effectiveness of treatments. Monitor
patient progress and adjust care plans as necessary. Provide
compassionate and efficient patient-centered care in a home
setting. Ensure compliance with health care laws, regulations, and
internal standards. Educate patients and their families on health
maintenance and disease prevention. Strong clinical skills in a
home health environment. Excellent organization and case management
abilities. Proficient in using medical software and telehealth
platforms. Ability to work independently and make decisions under
pressure. Exceptional interpersonal and communication skills.

401k Current Registered Nurse (RN) license in the state of
practice. Certified Case Manager (CCM) credential preferred.
